쇼그렌 증후군과 다른 결합조직질환의 구분, 인천이레한의원

쇼그렌 증후군과 다른 자가면역질환과의 연관성에 대해서 알아보겠습니다.

쇼그렌 증후군은 자가면역성 결합조직 질환(systemic autoimmune disease)에 속합니다.

루푸스 (Systemic Lupus Erythematosus, SLE)

류마티스 관절염 (Rheumatoid Arthritis, RA)

전신 경화증 (Systemic Sclerosis)

혼합 결합조직질환 (Mixed Connective Tissue Disease)

유육종증 (Sarcoidosis)

전신 혈관염 (Systemic Vasculitidies)

항인지질항체증후군 (Antiphospholipid Syndrome)

등이 쇼그렌증후군고 감별진단이 필요하고, 이런 질환에서 이차성으로 쇼그렌증후군이 나타나기도 합니다.

그 중에 특히 쇼그렌 증후군과 가장 유사하고 증상과 진단이 겹치는 자가면역질환은 SLE입니다.

위의 사진에서 보는 것과 같이, SLE의 유병률이 쇼그렌증후군(SS)보다 훨씬 높습니다.

SLE와 SS의 진단시에도 유사한면이 많이 있는데,

RF, anti-ssA, anti-ssB antibody가 공통적으로 존재하고

임상양상도 유사한 점이 있습니다.

SLE 환자중에 건조증을 나타내는 환자의 비율을 18-34%이고

SLE 환자중 쇼그렌증훈으로 진단되는 환자의 유병률이 9 - 19%1 2에 이른다고 합니다.

SLE 환자중 쇼그렌증후군을 같이 가지고 있는 환자들에게서는 SLE만 단독으로 가지고 있는 환자에게서 보다는

피로감과 혈소판 감소증이 높은 빈도로 나타났고, 대신 신증(nephropathy)은 적은 빈도로 나타났습니다.3

여러 연구를 통해서 분석해보자면

쇼그렌 증후군 환자 중 특히 관절염과 림프종을 가진 환자들은

anti-DNA 항체 검사를 추가적으로 실시한다면 SLE를 감별진단 할 수 있다고 합니다.4

류마티스 관절염 환자의 30- 50%가 건조증을 가지고 있고, 4-31%가 쇼그렌증후군으로 진단56됩니다.

전신경화증 환자의 67-68%가 건조증을 가지고 있고, 14-20%가 쇼그렌증후군으로 진단78 이 됩니다.

유육종증 환자의 9%가 건조증을 가지고 있고, 이 들 중 3%가 쇼그렌증후군으로 진단9 이 됩니다.

이런 식으로 다른 자가면역질환을 가진고 있던 환자가 쇼그렌증후군으로 다시 진단이 되면 이를 이차성쇼그렌증후군이라고 불렀지만 최근의 연구 동향은 일차성 이차성의 구분은 의미가 없다고 하는 추세입니다.

그 외에 MCTD환자의 42%10에서, 한랭글로블린현관염 환자의 30-53% 11 12가 건조증상을 가진다고 합니다.

따라서 입이 마르고, 눈이 건조하다고 해서 일단 쇼그렌증후군을 생각할 것은 아니고

다른 류마티스 질환도 같이 고려해야 할 것입니다.
